Effective January 1, 2015, Voluntary Vision Insurance is provided by Kansas City Life Insurance, Davis Vision.
Register online at www.davisvision.com to:
- Print a Membership Card (Click "Member" and register for a username and password if you don't already have one. Sign in and click on "Access Forms and Benefits", then Click on "Membership card" and you can print the ID card that displays)
- Locate a Provider
- Check Eligibility
- View Claim Status
